A computer system change at Anthem required new ID numbers for all participants effective March 1, 2022. All eligible participants should have the new ID cards by now. You and your eligible family members must use the new ID cards for all medical, dental, and vision services. For Medicare retirees, the new Anthem ID only applied to dental services. If claims are submitted with the old ID numbers, Anthem rejects them before they get to the Health Fund office. Please read the Explanation of Benefits (EOBs) closely to make sure all services are being processed through the Health Fund. Health Dynamics is terminating its annual wellness exam program as of December 1, 2022. Health Dynamics exams will be available and covered through November 30, 2022. As always, you can have wellness exams (annual physicals) from your primary care doctor. If you need help finding a primary care doctor in your area, go to www.anthem.com. Or call the Health Fund for assistance.

The new ID cards will be mailed soon directly from Anthem. You and your eligible family members must use the new ID cards as soon as you receive them to avoid claims being denied for missing the new ID number.
Effective January 19, 2022, every home in the U.S. can order 4 free at-home COVID-19 tests. The tests will be completely free. There are no shipping costs and you don’t need to enter a credit card number. Go to the website www.COVIDtests.gov.
If you chose to purchase COVID-19 tests from retail stores or on-line, the fund will reimburse eligible members in the health fund the cost of FDA approved, Over the Counter (OTC) COVID-19 tests. Be careful to shop at reputable on-line retailers to avoid scams. Read the attached FAQs for more information and select the link here or under the Documents tab for the claim form.
W-2 Forms: If you received Loss Of Time benefits in calendar year 2021 for a short term disability, you will receive a W-2 form for the amount of benefits paid to you by the Health Fund in 2021. Loss of Time benefits are taxable income for the IRS.
Form 1095-B: No forms are being mailed for 2021. While the Health Fund is required to report information to the IRS of the months that each of the participants and their dependents had health coverage in calendar year 2021, as an individual you are not required to report that information on your tax filing. If you need the form, please contact the Health Fund office.
If you lost health coverage with the Fund during April 1, 2021 through September 30, 2021, due to a reduction of your work hours, you may be eligible for a 100% COBRA subsidy to maintain your coverage with the Fund. Please review the attached Frequently Asked Questions. If you think you qualify for the free coverage during this period, contact the Health Fund office for the enrollment forms.
